Position Summary

The RN / Clinical Services Supervisor oversees and ensures the delivery of high-quality, evidence-based, safe, person-centered nursing and companion care services in compliance with state and federal regulations. This role supervises field staff (Nurses, RNs, CNAs, and sitters/companions), contributes to nursing policy and standard procedures for the organization, evaluates clinical documentation for accuracy and timeliness, provides training and evaluations, and acts as the lead clinical resource for the agency.

Schedule & Availability

  • Flexible scheduling options available (Full-time, Part-time, or Per Diem).
  • Core business hours: Monday–Friday, 9:00 AM – 4:00 PM for office and supervisory duties.
  • Occasional evening or weekend visits may be required depending on patient needs, staffing coverage, or regulatory requirements.
  • On-call availability may be expected for urgent clinical questions or regulatory inspections.

  • Salary: Commensurate with experience and competitive with the North Carolina healthcare market.

Key Responsibilities

Patient Assessment & Care Planning

  • Conduct initial and ongoing comprehensive assessments of new patients (physical, psychosocial, environmental, functional).
  • Develop individualized care plans; revise as conditions change.
  • Identify risk factors, complications, and special care needs in home, hospital, or nursing facility settings/assisted living facilities. 
  • Clearly explain care plans to families/caregivers and provide education on treatments, safety, and home care needs.
  • Reviews, authors, implements, and periodically updates nursing policies and procedures to reflect current best practices and regulations.
  • Maintains comprehensive knowledge of and ensures compliance with all Federal, State, and local regulations pertaining to home health and care guidelines for Commercial and Private health plans as appropriate. 
  • Develops, implements, and monitors quality assurance and performance improvement initiatives to enhance patient outcomes and satisfaction.
  • Stay up to date with changes in home health regulations and share updates with the team.
  • Prepares for and participates in any regulatory or accreditation surveys and contributes to the general knowledge of clinical and nursing care in the organization. 


Clinical Oversight

  • Monitor and evaluate patient care to ensure adherence to the care plan and regulatory standards.
  • Review documentation for accuracy and completeness.
  • Perform supervisory visits and audits as required by state regulations and agency policy.
  • Administer non-medical treatments safely in accordance with physician orders and agency policy.
  • Monitor patients and report changes promptly.
  • Educate patients and families on safety.
  • Delegate medication-related tasks to qualified staff in line with NC Board of Nursing rules.


Staff Supervision & Training

  • Directly supervise RNs, CNAs, and companion staff in the field.
  • Train CNAs and care giver in patient care tasks, safety procedures, documentation, and infection control.
  • Delegate tasks appropriately within scope of practice and competency.
  • Conduct performance evaluations, competency checks, and ongoing education.
  • Provide coaching, feedback, and corrective action when needed.


Care Coordination

  • Participate in case conferences to develop and update care plans.
  • Communicate with physicians, clients, and families regarding clinical status and care needs.
  • Ensure compliance with HIPAA, OSHA, and agency policies.


Regulatory Compliance & Quality Assurance

  • Maintain knowledge of NC Board of Nursing rules and federal/state home care regulations.
  • Ensure compliance with NC DHSR rules (10A NCAC 13J), including:
    • RN completion of initial and periodic patient assessments.
    • RN development and review of all care plans.
    • Continuous RN availability while CNAs are providing care.
    • Supervisory visits to evaluate staff performance and patient safety.
    • Verification of CNA registry status and scope of practice compliance.
  • Prepare for state surveys and audits; participate in internal quality reviews.
  • Maintain staff training, competency evaluations, and performance documentation.
  • Ensure compliance with NC DHSR requirements, including RN assessments, 90-days supervisory reviews, and staff oversight.
  • Be available to participate in state inspections, surveys, and audits, representing the agency’s clinical services and providing documentation or clarification as requested by regulators.
  • Assist in preparing for inspections by ensuring records, policies, and staff files are accurate and up to date.


Other Duties

  • Maintain current RN license, BLS/CPR, and other required certifications.
  • Travel to patient homes and facilities as needed.
  • Stay current on North Carolina nursing regulations and agency licensing requirements.
  • Accompany patients to medical or hospital appointments when skilled nursing oversight is required.
  • Communicate directly with physicians and healthcare providers to clarify orders, document instructions, and update the plan of care.
  • Ensure that CNAs who accompany patients to appointments are properly trained, delegated, and supported within their scope of practice.
  • Provide follow-up education and guidance to patients and families regarding medical recommendations.


Qualifications

  • Current, unrestricted RN license in North Carolina.
  • Minimum 3 years of clinical nursing experience (home health or facility preferred).
  • Supervisory/leadership experience strongly preferred.
  • Knowledge of NC Nursing Practice Act, Board of Nursing delegation/supervision rules, HIPAA, and OSHA.
  • Strong organizational, communication, and critical-thinking skills.
  • Strong understanding of patient rights and regulatory compliance requirements.
  • Valid driver’s license and reliable transportation.


Physical Requirements

  • Ability to travel to client homes and facilities.
  • Ability to lift/move up to 25 lbs. and assist with patient mobility

Work Environment

  • Combination of office and field work, with exposure to varying patient conditions.
  • Occasional evening or weekend availability may be required based on staffing needs.
